چکیده

Ten young adult cats, 6 intact males and 4 intact females, with reduced renal mass were randomized to receive either normal feline maintenance diet alone or diet plus phosphorus binder in a crossover design with each treatment period of 56 days in duration (Dietary Trial A). For Dietary Trial B, following a 3-month withdrawal period, 6 cats were placed on the intestinal phosphorus binder for 9 months to determine the longterm effects of this approach. During both Dietary Trials A and B, cats were studied by serum and urine biochemical testing and renal clearance studies. In Dietary Trial A, compared to diet alone, serum phosphorus and plasma parathyroid hormone concentrations were significantly lower when intestinal phosphorus binder was provided. There were no significant differences in blood urea nitrogen, serum creatinine concentration, glomerular filtration rate, or renal plasma flow. In Dietary Trial B, serum phosphorus and plasma parathyroid hormone concentrations were reduced at the 6and 9-month time points of the treatment period compared to measurements obtained before intestinal phosphorus binder administration. We conclude that the addition of an intestinal phosphorus binder to a normal maintenance feline diet can lower serum phosphorus and parathyroid hormone concentrations in cats with International Renal Interest Society stages I and II chronic kidney disease. The effect of the binder to reduce serum phosphorus concentration was present by Day 56 in Dietary Trial A and persisted at 6 and 9 months in Dietary Trial B.

Serum profiles of calcium, phosphorus, magnesium, vitamin D and parathyroid hormone in Caspian horses during different seasons

Minerals play an essential role in the normal vital process.Calcium, phosphorus and magnesium are the most abundant minerals. Vitamin D and parathyroid hormone play a key role in regulation of their homeostasis as well. The aim of this study was to evaluate calcium, phosphorus, magnesium, vitamin D and parathyroid hormone on 30 Caspian horses of Guilan Province (south of the Caspian Sea) in two...

Phosphorus and Nutrition in Chronic Kidney Disease

Patients with renal impairment progressively lose the ability to excrete phosphorus. Decreased glomerular filtration of phosphorus is initially compensated by decreased tubular reabsorption, regulated by PTH and FGF23, maintaining normal serum phosphorus concentrations. There is a close relationship between protein and phosphorus intake. In chronic renal disease, a low dietary protein content s...

The challenge of controlling phosphorus in chronic kidney disease.

The pathogenesis and management of chronic kidney disease-mineral bone disorders (CKD-MBD) has experienced major changes, but the control of serum phosphorus at all stages of CKD still seems to be a key factor to improve clinical outcomes. High serum phosphorus is the most important uremia-related, non-traditional risk factor associated with vascular calcification in CKD patients and in the gen...

Sevelamer hydrochloride, a calcium-free phosphate binder, inhibits parathyroid cell proliferation in partially nephrectomized rats.

BACKGROUND Secondary hyperparathyroidism characterized by hyperplasia of the parathyroid gland (PTG) is a consequence of chronic renal insufficiency (CRI). Dietary phosphate restriction and sevelamer hydrochloride, a calcium-free phosphate binder, suppress parathyroid hormone (PTH) secretion and PTG hyperplasia in experimental animals with CRI, independently of serum calcium and 1,25(OH)(2)D(3)...

Long-term treatment of secondary hyperparathyroidism with the calcimimetic cinacalcet HCl.

BACKGROUND Patients with secondary hyperparathyroidism often require therapy that provides long-term control of parathyroid hormone concentrations without increasing calcium and phosphorus concentrations. Cinacalcet modulates the calcium-sensing receptor on the parathyroid gland to reduce secretion of parathyroid hormone and lower serum calcium, phosphorus and calcium-phosphorus product in haem...
